WELLINGTON - A Japanese restaurant inside The Mall at Wellington Green was closed briefly last week after failing state inspections.


Tokyo San at 10300 Forest Hill Blvd. was closed Aug. 29 after inspectors with the Florida Department of Business and Professional Regulation found live and dead roaches, rusted shelves and stained equipment, among other violations, according to records.


Inspectors found seven violations during their inspection, including two that were high priority. One intermediate violation and four basic violations also were found.

High-priority violations are those that could contribute directly to a food-borne illness or injury, and include items such as cooking, reheating, cooling and hand-washing.


The restaurant met inspection standards on a follow-up visit.


Ten Palm Beach County restaurants had perfect inspections last week. They are Moon’s Tavern in Boca Raton, Wendy’s Old-Fashioned Hamburgers at 9192 Glades Road in Boca Raton, KFC at 360 N. Congress Ave. in Boynton Beach, Checker’s at 2270 S. Military Trail in West Palm Beach, Chick-Fil-A at 2070 Palm Beach Lakes Blvd. in West Palm Beach, Christ Fellowship Church Food Truck in Palm Beach Gardens, Russo’s Submarine Sandwich in West Palm Beach, Hugo’s Gourmet Catering in Riviera Beach, Ironhorse Country Club Snack Bar in West Palm Beach, and Oasis Ice Cream in West Palm Beach.
